What does a questionnaire look like in Hora Finita?

General information:

Questionnaires are used by some Graduate Schools to monitor and evaluate the progress of a PhD candidate's project. Questionnaires are usually sent alongside a checkpoint such as the yearly evaluation checkpoints. 
Faculties/Graduate Schools use questionnaires for PhD candidates and in some cases their supervisors.

When a checkpoint is coming up the PhD candidate will receive a reminder email in advance, informing the PhD candidate of any questionnaires accompanying the checkpoint.

There are several different standard questionnaire types already in Hora Finita. Faculties/Graduate schools may add/alter questionnaires depending on their needs and preferences.

What are confidential questionnaires?

Some questionnaires such as the 'Supervision Evaluation Questionnaire' are marked as confidential. This means that only the intended respondent can fill in the questionnaire and only the Graduate School Coordinator may view the response.

If a questionnaire is not marked as confidential, the responses can be viewed by both the PhD candidate and the supervisor.

Where can I find my questionnaires?

Option 1: Tasks section

Upcoming questionnaires will be listed under the task section on the dashboard. Click on the questionnaire to go directly to it's page.

Option 2: Via the checkpoint

Navigate to the checkpoint to which the questionnaire is linked.

Click on a specific questionnaire to go to it's page.

How do I complete a questionnaire?

After you have filled in your questionnaire, scroll down to the bottom of the page and press 'Save and go back' to save and finish the questionnaire.

Mandatory questions are indicated by a *. If you have not filled in all mandatory fields you will be unable to save the questionnaire.
